In 1949, to help stem the spread of tuberculosis, the city of Seattle created a locked ward for TB sufferers who deny treatment. The ward became a model for other cities.

Seattle health officials and the staff of Firland Sanatorium instituted an informal policy that kept all Skid Road alcoholics hospitalized for 12 months of supervised antibiotic therapy, roughly 6 months longer than the average stay.
